FAQ

  • Do I need a prescription from my doctor? A doctor’s referral or prescription is not required for most plans (excluding Medicare which requires a doctor’s prescription).   This means you can attend physical therapy without having to see your physician first.  I will assess at the initial and subsequent sessions if I determine it would be beneficial for you to see a physician.   Please contact our office if you have any questions about your insurance plan’s requirements.  I can also recommend a physician for your condition if requested.
  •  Do you take insurance?  I am currently only in network with BCBS PPO.  As a courtesy I will contact your insurance provider for a quote on coverage details for in-network and out-of-network insurance coverage.
  •  Do you accept self-pay?  Yes.  Our self-pay rate is $175 for the initial session and $150 for subsequent sessions.  I can also provide you with a superbill to submit to your insurance.
  • How many visits will I need to attend?  Each patient’s condition is unique. We will collaborate (patient, PT and referring physician if applicable) to determine an effective plan of care that takes into consideration your diagnosis, your goals, your time as well as finances.
